VBS 2026 RELEASE OF LIABILITY & CONSENT FOR TREATMENT - I/We do hereby release, discharge and/or indemnify Calvary Chapel West Houston, it's staff, volunteer and agents from any liability whatsoever arising out of any injury, illness, damage, harm or loss of any kind which may be sustained by said person(s) during the course of participation/involvement with Calvary Chapel West Houston In case of Injury/ emergency or loss of ability to communicate, I/We consent to the treatment by any medical emergency service representative, including the transportation by an emergency vehicle to a hospital or medical facility. I/We herby authorize and consent to the performance of medical examination, treatment and hospital care from any physician, nurse or medical personnel, including but not limited to x-rays, anesthesia and surgery. -
